QUALIFICATIONS: Bachelors degree in Social Work, Psychology, or Gerontology or advanced professional training in relevant discipline with experience and/or training with the elderly and/or mentally or physically disabled required. Good community networking skills, public speaking skills and written and oral communication skills required. Familiarity with personal computer word-processing and databases preferred. Must possess a valid driver's license, automobile insurance, and access to an automobile to perform program related duties.

 

GENERAL DUTIES:

This casework position is responsible to deliver high quality Guardianship and Power of Attorney (POA) services on behalf of incapacitated adults. Regular and predictable attendance is an essential function of this position. Under the direct supervision of the Guardianship Supervisor, the Guardianship Caseworker will have the following specific responsibilities:

 

a) Provide substitute decision making services in the areas affecting physical, emotional, shelter, financial and medical needs for clients deemed incapable to manage their own welfare and in need of a guardian by the Courts of the petitioning County.

b) Visit clients not less than every six to eight weeks unless the client situation and weather conditions indicate otherwise, liaison with personnel and/or family supervising the clients' living environment, and participate in appropriate care planning meetings to assure the welfare of the client.

c) Liaison with the referring County or private petitioning agency and attend court hearings as required.

d) Manage bookkeeping activities for Guardianship clients’ accounts producing auditable records of activities; apply for and manage client benefits; establish funeral accounts and arrangements.

e) Demonstrate a current knowledge of services and resources in the community, skill in making appropriate referrals to those services and resources, and the ability to establish and maintain effective working relationships with other community providers and consultants to the benefit of the client.

f) Maintain high standards of professionalism including a commitment to protecting client confidentiality and rights to the inclusion and the least restrictive setting at all times.

g) Maintain accurate client service records, to include reporting data needed to complete the Guardianship program’s quarterly quality assurance and outcomes reports.

h) Participate in an on-call after hours coverage for crisis and emergent situations effecting clients of the guardianship programs.

i) Demonstrate a current knowledge of the mandates, laws, and practice standards effecting guardianship services in Pennsylvania, and national standards of practice.

 

Salary is $38,000/year with benefits that start on the first day of employment.  Agency pays 100% for employee premiums for health, dental, vision and life insurance.  We also offer a 401(k) and paid sick, annual, personal days and holidays.
