Responsible to implement and provide individual and family treatment consistent with treatment plan and identified therapeutic needs of individual(s), using up-to-date therapy methods/models as assigned by supervisor.
Conduct initial evaluations of clients’ mental health problems and formulate a diagnosis and treatment plan, consistent with clinical standards.
Assist in making reminder calls/texts for intake appointments and PEs, as appropriate.
Conduct sessions with individuals receiving Medication Management Services to develop, review and update Medication Management treatment plan and to assess current needs and services needed.
Serve as clinical liaison between the client and the medical team.
Document all client services/contacts in client record.
Complete all necessary clinical documentation, assessments, treatment plans and reviews, and evaluations/summaries in a timely manner following program procedures.
Complete necessary documentation required for managed care/Insurance authorizations and reauthorizations.
Gather necessary documents and information from clients/families to complete liability documentation.
Follow the liability submission to the LSBHIDEI office to the point of approval.
Identify LSBHIDEI as the payor and inform necessary staff to acquire authorization.
Participate in staff/clinical team meetings, supervision, and case reviews for administrative purposes and the development and implementation of client treatment.
Refer individuals for additional or alternative services, as indicated.
Advocate for clients, their rights and assist clients in ensuring their rights are protected and preserved.
Enter and maintain daily schedule into computer as required by departmental procedures.
Comply with Agency policies and procedures, including those regarding conduct, confidentiality, and record keeping.
Perform duties in a professional manner in response to clients, co-workers, and situations in which own values are challenged.
