  • Responsible to implement and provide individual and family treatment consistent with treatment plan and identified therapeutic needs of individual(s), using up-to-date therapy methods/models as assigned by supervisor.
  • Conduct initial evaluations of clients’ mental health problems and formulate a diagnosis and treatment plan, consistent with clinical standards.
  • Assist in making reminder calls/texts for intake appointments and PEs, as appropriate.
  • Conduct sessions with individuals receiving Medication Management Services to develop, review and update Medication Management treatment plan and to assess current needs and services needed.
  • Serve as clinical liaison between the client and the medical team.
  • Document all client services/contacts in client record.
  • Complete all necessary clinical documentation, assessments, treatment plans and reviews, and evaluations/summaries in a timely manner following program procedures.
  • Complete necessary documentation required for managed care/Insurance authorizations and reauthorizations.
  • Gather necessary documents and information from clients/families to complete liability documentation. 
  • Follow the liability submission to the LSBHIDEI office to the point of approval. 
  • Identify LSBHIDEI as the payor and inform necessary staff to acquire authorization.
  • Participate in staff/clinical team meetings, supervision, and case reviews for administrative purposes and the development and implementation of client treatment.
  • Refer individuals for additional or alternative services, as indicated.
  • Advocate for clients, their rights and assist clients in ensuring their rights are protected and preserved.
  • Enter and maintain daily schedule into computer as required by departmental procedures.
  • Comply with Agency policies and procedures, including those regarding conduct, confidentiality, and record keeping.
  • Perform duties in a professional manner in response to clients, co-workers, and situations in which own values are challenged.
