JOB PURPOSE: Promotes and restores our client’s health, wellness, recovery & use of Personal Medicine & self-management tools.

Responsibilities: (pursuant to assignment)

  • Provides clinical consultation/interface with Physical Health and Behavioral Health clinicians and members and their families.
  • Assures completion of wellness physical assessment on each identified client.
  • Coordinates and collaborates with Primary Care Physician and specialty medical providers.
  • Use critical reasoning and clinical judgment to assess the needs of clients.
  • Engaging individuals in recovery in ongoing wellness coaching based on the eight domains of wellness.
  • Assist individuals and their families to identify strengths to achieve wellness goals.
  • Outcomes monitoring as requested/required by CCBH.
  • Participate in initial and ongoing wellness coaching training.
  • Participate in Wellness Nurse support calls as requested/required.
  • Participate in case conferences to support best practice as requested/required.
  • Promote the Integration of wellness and recovery principles.
  • Attend and participate in the Annual BHHP conference
  • Set a SMART goal and create action plan for identified/referred clients.
  • Monitors disease registry to check progress for each person participating
  • Supervises wellness coach process by health navigators.
  • Supervises wellness coach process by health navigators.
  • Advocate for unmet needs of clients.
  • Accompany individual to community appointments as indicated.
  • Assist in the creation of environments that promote wellness and resiliency practices.
  • Complete all documentation in a timely, comprehensive and legible manner and in accordance with Friendship House policies and procedures.
  • Contribute to any quality improvement/outcome measurement tracking and data gathering as assigned.
  • Perform duties in a professional manner in response to clients, co-workers, and situations in which own values are challenged.
  • Other duties as assigned.

Qualifications

  • Registered Nurse in active license in good standing in Pennsylvania.
  • 2 years of direct patient care experience required. Mental health experience preferred.
